Leaving the Hospital Is More Than Just Going Home
After more than 20+ years as a Registered Nurse, one lesson has stayed with me throughout my career: caring for patients doesn't end when they're discharged from the hospital. Early in my nursing career, I realized that discharge planning doesn't end when the paperwork is complete. Many patients are medically ready to leave the hospital but still face practical challenges getting home safely. Some have supportive family members waiting for them, while others worry about climbing stairs, getting into a vehicle, or simply making it through the front door. Those moments reminded me that transportation is part of the patient's healthcare journey—not separate from it. As nurses, we often focus on preventing falls, reducing complications, and ensuring a safe recovery. Those same goals should continue from the hospital door all the way home.
I've cared for patients in critical care, procedural areas, and recovery units. Many were excited to finally hear the words, "You're going home today." But even after receiving excellent medical care, I often found myself thinking about what happened next.
Who is taking them home?
Will they get there safely?
For some patients, the answer is simple. A family member is waiting outside, ready to help. But for many others, getting home can be one of the most stressful parts of the entire healthcare journey.
Every Patient's Situation Is Different
Not everyone leaving the hospital is recovering under the same circumstances.
Some patients have just undergone surgery and are still experiencing pain or the effects of anesthesia. Others may feel weak after several days in the hospital. Older adults may have balance issues, use walkers or wheelchairs, or simply need an extra hand getting into a vehicle.
I've also cared for patients whose loved ones desperately wanted to help but couldn't leave work, were caring for children at home, or lived too far away to be available.
These are everyday situations that healthcare professionals encounter, yet transportation is often one of the last pieces of the discharge plan.
Safe Transportation Is Part of Safe Patient Care
As nurses, we spend our shifts helping patients prevent falls, manage pain, understand discharge instructions, and prepare for recovery.
Those same principles don't suddenly stop at the hospital entrance.
A safe trip home means considering questions such as:
Can the patient safely get into and out of the vehicle?
Will someone be available to assist when they arrive home?
Does the patient need extra time because of limited mobility?
Is the transportation reliable and appropriate for their condition?
Sometimes, what seems like a simple ride home can make a meaningful difference in a patient's comfort, confidence, and recovery.
Compassion Matters Just as Much as Reliability
Transportation is often the first experience a patient has after leaving the hospital.
A kind greeting, patience while walking, assistance with belongings, and understanding that recovery takes time can make someone feel respected during a vulnerable moment.
As healthcare professionals, we know that compassion isn't measured only by medical treatments. Sometimes it's reflected in small moments—offering reassurance, moving at the patient's pace, or simply recognizing that this may be an emotional day for them and their family.
Those moments matter.
Why I Started AccessCare Pathways
AccessCare Pathways was founded because I wanted to bring a healthcare perspective to transportation.
As a Registered Nurse, I've spent my career advocating for patients, promoting safety, and supporting families during difficult times. Those values continue to guide how I envision transportation services should be delivered—with professionalism, compassion, reliability, and respect.

Every patient deserves to feel safe—not only while receiving medical care, but also during the journey home.
As healthcare professionals, we often speak about continuity of care. To me, continuity of care includes every transition a patient experiences, including transportation after discharge.
